OGE Form 450 - CONFIDENTIAL FINANCIAL DISCLOSURE REPORT
To be used by employees graded 15 or below, Special Government Employees, or those not covered under the Super Grade pay plans.
OGE 450A - Confidential Certificate of No New Interests -- to be used only when there have been no changes from filing your last 450. This form may not be used by SGEs (Collaborators, Consultants, Experts, or Panel Managers) or by first time filers. This form is not available for use during Calendar Year 2008. Only form OGE-450 will be accepted this year.
SF-278 - PUBLIC FINANCIAL DISCLOSURE REPORT
(For the complete form, please print Page 1, and Schedule A through Schedule D. This form is to be used only by employees in the following pay plans: SES, RA, SL, and ST; or by employees paid 120% of the GS-15 level.)
